Pfizer and BioNTech have asked the Food and Drug Administration for emergency use authorization of their COVID-19 booster targeting omicron subvariants BA.4 and BA.5 and the original strain of the virus from 2020 for children 6 months through 4 years.

Children within that age group would receive the omicron boosters as the third shot in their primary series if approved, the vaccine manufacturers said.

The primary series for children younger than 5 currently includes three doses of the original COVID-19 vaccine, while the primary series for those over 5 only involves two doses.

“With the high level of respiratory illnesses currently circulating among children under 5 years of age, updated COVID-19 vaccines may help prevent severe illness and hospitalization,” Pfizer and BioNTech said Monday.

The request comes as a surge in respiratory illnesses, including the flu and respiratory syncytial virus, or RSV, has led to overcrowding in some children’s hospitals across the country.

Pfizer and BioNTech’s omicron booster has already been authorized for people 5 and older in the United States and the European Union.

Meanwhile, the emergence of new coronavirus subvariants BQ.1 and BQ.1.1 might affect the effectiveness of the updated boosters. Pfizer-BioNTech said last month that its updated booster still offers more protection against newer omicron subvariants, including BQ.1.1, than its original COVID-19 vaccine.

These “variants” are not real.

Dozens or hundreds of new strains of the flu appear every year.

COVID-19 vaccines will not be enough to withstand the Omicron variant, warned the CEO of BioNTech, the German company behind the mRNA vaccine produced with Pfizer.

“We must be aware that even triple-vaccinated are likely to transmit the disease…It is obvious we are far from 95 per cent effectiveness that we obtained against the initial virus,” Ugur Sahin told the French daily Le Monde in an interview published on Monday.

Despite this, Sahin said early data from Britain and South Africa is providing us with “reassuring information”.

The latest research from South Africa, where Omicron was first reported, suggested that two doses of the Pfizer/BioNTech vaccine offered 70 per cent effectiveness in reducing the risk of hospitalisation.

“There will be a loss of effectiveness against Omicron over time, it’s very likely, but it’s still to be measured how quickly. I will not base predictions on preliminary laboratory data but on real-life data, which is much more appropriate,” the German immunologist said.

Sahin said testing for COVID-19 is important, especially for the elderly and during winter, as are other protective measures such as mask-wearing, adding, “otherwise we will not be able to control the rapid expansion of this new variant.”

The German company is already designing a coronavirus vaccine adapted to the new variant, using Omicron’s spike protein and its 32 mutations as an antigen. Sahin announced this should be ready by March.

New measures are coming after Christmas, in January.

BioNTech says it could produce and ship an updated version of its vaccine within 100 days if the new Covid variant detected in southern Africa is found to evade existing immunity.

The German biotechnology company is already investigating whether the vaccine it developed with US drugmaker Pfizer works well against the variant, named Omicron, which has caused concern due to its high number of mutations and initial suggestions that it could be transmitting more quickly.

The company says it will know in two weeks whether its current vaccine is likely to be sufficiently effective against the B.1.1.529 variant, now named Omicron by the World Health Organization, based on lab-based experiments.

If required, BioNTech said it is poised to tweak its vaccine to match it more closely to the new variant.

“Pfizer and BioNTech have taken actions months ago to be able to adapt the mRNA vaccine within six weeks and ship initial batches within 100 days in the event of an escape variant,” the company said in a statement.

The US company Moderna said it was testing three existing Covid-19 vaccine booster candidates against the Omicron variant, and that it would develop a new variant-specific booster against it.

“From the beginning, we have said that as we seek to defeat the pandemic, it is imperative that we are proactive as the virus evolves. The mutations in the Omicron variant are concerning and for several days, we have been moving as fast as possible to execute our strategy to address this variant,” said Stéphane Bancel, Moderna’s chief executive.

Other vaccine teams, including Johnson & Johnson, also confirmed on Friday that they were testing the effectiveness of their vaccines against the new variant to assess whether updates were likely to be required. AstraZeneca said it is already conducting research in Botswana and Eswatini, where the variant has been identified, to collect real-world data on how the Oxford/AstraZeneca vaccine performs against the new variant.

At the moment, concerns about a decline in protection are theoretical based on the very high number of mutations – double that seen in Delta – on the spike protein that the vaccine targets.

A new vaccine “tailored” to target probable future mutations of the Covid-19 coronavirus will likely be needed by the middle of 2022, according to the head of BioNTech, the biotechnology company that developed the Pfizer shot.

The German firm’s CEO, Uğur Şahin, told the Financial Times that Covid-19 variants such as the Delta strain were more contagious but could be tackled by booster shots of current vaccines.

However, he added that mutations would emerge that could slip past the body’s vaccine-acquired immune defenses.

“This year, [a different vaccine] is completely unneeded. But by mid next year, it could be a different situation,” Sahin said, adding that “tailored” versions of current vaccines would be needed to specifically target the new strains that emerge.

“This virus will stay, and the virus will further adapt,” he said.

“We have no reason to assume that the next-generation virus will be easier to handle for the immune system than the existing generation. This is a continuous evolution, and that evolution has just started.”

Vaccination programs will feature two main streams by next year, Sahin predicted, with booster shots for those already inoculated and a continued campaign to get the vaccines to those who have had minimal access to them thus far.

The US Food and Drug Administration is preparing to authorize the Pfizer/BioNTech Covid vaccine for adolescents between ages 12 and 15 years by early next week, the New York Times reported on Monday, citing federal officials familiar with the agency’s plans.

An approval is highly anticipated after the drugmakers said in March that the vaccine had been found to be safe, effective and produced robust antibody responses in 12- to 15-year-olds in a clinical trial.

Responding to a Reuters request for comment, the FDA said its review of expanding the vaccine’s emergency use authorization was continuing, but it did not provide further details.

The vaccine has already been cleared in the United States for people age 16 and above.

Pfizer and Moderna have also launched trials in even younger children, from six months to 11 years old. Both companies have said they hope to be able to vaccinate children under 11 as soon as early 2022.

BioNTech’s Covid-19 vaccine, jointly developed with Pfizer, should be available to 12- to 15-year-olds in Europe from June, the chief executive of the German company has said.

Uğur Şahin said BioNTech had submitted its application for emergency approval of vaccines for this age group in the US at the start of April, and next Wednesday would be ready to submit the application for approval by the European Medicines Agency.

The Pfizer/BioNTech vaccine is already approved in the US and the EU for those 16 years of age and older. Vaccinating children is seen as a crucial next step towards herd immunity and ending the pandemic.
